Collar Waiver

I understand that Walkers Pet HoTail has a policy of removing collars (including, but not limited to, flea collars) and harnesses from pets in their care in order to avoid any injuries.

I understand and accept that Walkers cannot guarantee the health of any animal and I demonstrate my good faith in Walkers and its staff by leaving my pet(s) in their care.

I hereby state that I do NOT want my pets’ collar/harness removed for its boarding/daycare stay at Walkers, and assume all liability in the event of injury to my pet(s), another pet in the care of Walkers or damage/loss to said collar, harness, tags, etc.

Owner agrees all services are provided under the terms of the Customer Agreement and Waiver.

This waver remains in effect for all future visits unless you notify Walkers in writing of any changes.

If signed electronically, by typing your name above, you agree the same constitutes an electronic signature executed with the intent to sign and to be bound by these terms and conditions.
